  • Requirements: The following documents are required along with a completed registration form in order to enroll a child in Sacramento City Unified School District:1. Photo Identification: State Issued ID, or other photo identification (work badge, passport, student ID, Costco card).2. Address Verification (the name on the ID must match the name on the bill issued within 30 days):a. Utility service contract, statement, or payment receipts b. Property tax payment receiptsc. Rental property contract, lease with payment receiptsd. Voter registratione. Correspondence from a government agencyf. Pay stubsIf an individual does not have the ability to provide proof of address:a. If living in a Primary/Permanent Residence, individuals must submit a completed and signed Address Affidavit with the required documentation.b. If an individual is unhoused/homeless, they must submit a completed and signed Housing Survey.3. Proof of Birth: Local registrar or county issued birth certificate, duly attested baptism certificate, or passport. If an individual does not have the ability to provide proof of birth, the parent/guardian may provide any other appropriate means of proving the age of the child.4. Immunization Record: current for each child.5. T-Dap Booster shot is required for grades 7th-12th. Both name and date of birth must be on the immunization record.6. Current Withdrawal Grades and Transcripts: required for grades 9th-12th.7. Individualized Education Plan (IEP): documentation for receiving Special Education Services, if applicable.8. Guardianship/Custody Documents: if applicable.
